Give every request a separate row
Record the request ID, amount, method, destination, creation time and status for each open payout. Keep the combined total beside the eligible balance to prevent accidental duplication.
Do not treat the oldest request's approval as proof that the other two passed review.

Use the open slots deliberately
Before adding a second or third request, confirm the account and method limits and whether KYC is complete. If the first request is blocked, resolve that stage before multiplying the same problem.

Close requests only on receipt
After dispatch, attach the processor reference and destination credit to the matching row. Mark a request closed only when the owned account controls the funds or a return is documented.
If one request disappears from the list, ask for its final status rather than assuming it merged with another payout.
